Allergy Shots Schedule

MONDAY, TUESDAY , WEDNESDAY, FRIDAY 8:00 AM TO 4:30 PM

THURSDAY 8:00 AM TO 5:00 PM

If you arrive by 4:30 PM (5:00 PM on Thursdays) you can be given your allergy shots.  We offer lunch time hours for your convenience, 5 days a week.  Any adjustments in these hours will be posted at the office and on the website.  It is our intent to post these changes at lease a month in advance where possible.

The office is usually closed the week after Christmas and injections will not be given during that week.  Normal business hours will resume on January 2 or the next business day following New Year’s Day.  The week of the Christmas holidays there are usually adjustments made in the schedule.  Please contact the office for this information.

Please select this link for the immunotherapy packet and consent form.  Please review them carefully.  If your allergist recommended this treatment for you specifically and you have decided to proceed with immunotherapy, simply sign the forms and deliver them to the office.  You may mail or fax them as well.  We suggest checking with your insurance company prior to initiation of allergy injections, as policies vary regarding coverage for allergen immunotherapy.  Once these forms are received, it signifies to us that you have read all of the information and are familiar with your insurance coverage and your customized immunotherapy vaccines will be prepared for you.
